How to File a Witness Slip

Witness Slip

Please follow the below instructions for filing as a proponent, opponent, or neutral on legislation that goes through the Illinois General Assembly

  1. Click on the link provided in the email or online.
  2. Fill out the Identification section with your information. For the Firm/business or agency and Title fields, write “self” unless you are authorized by a company or business to use their name on the witness slip.
  3. The Representation section is optional. Fill in the Representation section only if you are appearing on behalf of a group, organization, or other entity. If you are not authorized to appear on behalf of any group, leave this section blank or fill in N/A or “self”.
  4. Complete the Position section. Select your position on the legislative item. You may file as a proponent (in favor/support of) or an opponent (in opposition to/against) of the piece of legislation. If there is an amendment to the bill, you may need to select the amendment from the menu in this section.
  5. Complete the Testimony section. Select the testimony types that you will supply for the hearing. If you do not intend to offer oral testimony or file a written statement, check the Record of Appearance Only box. This will allow your position on the piece of legislation to be known without having to physically appear.
  6. In Section 4, beneath the buttons, is CAPTCHA text. The term CAPTCHA stands for “Completely Automated Public Turing Test To Tell Computers and Humans Apart.” Type what is displayed in the CAPTCHA box in the field below the box. If you register and log in, you will not get the CAPTCHA prompt.
  7. Click in the box next to “I Agree to the ILGA Terms of Agreement”.
  8. Click Create (SLIP).
  9. You are done!
